You will lose eligibility as a Voluntarily and Continuously Insured Person only in the cases indicated below. Since procedures differ for each case, refer to the applicable procedures below.
(1)Two years have passed since the date you became a Voluntarily and Continuously Insured Person
(2)Death of the insured person
(3)Failure to pay insurance premiums by the designated deadline
(4)If you became an insured person under another health insurance plan after starting new employment
(5)If you became an insured person under the Medical Care System for the Advanced Elderly
(6)If you applied to have your status as a Voluntarily and Continuously Insured Person cleared, the last day of the month including the date on which the application was received
- *Note:If you hope to enroll in the new insurance system on April 1, the application must have arrived (accepted) at the Health Insurance Association between March 1 and the end of the month.
Note
If you will no longer be a Voluntarily and Continuously Insured Person, be sure to return your health insurance card, Certificate of Application of Maximum Copayment Amount, elderly benefits card, etc. to Rakuten KENPO.
(1) Two years have passed since the date you became a Voluntarily and Continuously Insured Person
- Rakuten KENPO will send a Certificate of Loss of Eligibility to your registered address when the date of loss of eligibility approaches.
- Please return your health insurance card to Rakuten KENPO on or after the day after the date of loss of eligibility.
If you were issued any other documents by Rakuten KENPO (such as an elderly benefits card or a Certificate of Application of Maximum Copayment Amount), return them along with the health insurance card. - Using the Certificate of Loss of Eligibility, complete the procedures necessary to join another health insurance plan.
- *Check with your new insurer (National Health Insurance or another health insurance society) regarding application procedures.
(2) Death of the insured person
- Download and print the following form from the website and complete it, referring to the example.

Example - Send the following three documents to Rakuten KENPO:
- Application Form for Disqualification as Health Insurance Voluntarily and Continuously Insured Person
- Health insurance card(s) (including those of any dependents)
If you were issued any other documents by Rakuten KENPO (such as an elderly benefits card or a Certificate of Application of Maximum Copayment Amount), return them along with the health insurance card. - Documentation of death (for example, a copy of the death certificate)
- *Note: Dependents can no longer use their health insurance cards from the day after the death of the insured person.
- Rakuten KENPO will send a Certificate of Loss of Eligibility to your registered address.
- For dependents: Using the Certificate of Loss of Eligibility, complete the procedures necessary to join another health insurance plan.
- *Check with your new insurer (National Health Insurance or another health insurance society) regarding application procedures.
(3) Failure to pay insurance premiums by the designated deadline
- Rakuten KENPO will send a Certificate of Loss of Eligibility to your registered address.
- After you receive the Certificate of Loss of Eligibility, promptly send the following:
- Health insurance card(s) (including those of any dependents)
If you were issued any other documents by Rakuten KENPO (such as an elderly benefits card or a Certificate of Application of Maximum Copayment Amount), return them along with the health insurance card.
- Health insurance card(s) (including those of any dependents)
- Using the Certificate of Loss of Eligibility, complete the procedures necessary to join another health insurance plan.
- *Check with your new insurer (National Health Insurance or another health insurance society) regarding application procedures.
*Note: Your eligibility will be cancelled in such cases pursuant to Article 38 of the Health Insurance Act.
(4) If you became an insured person under another health insurance plan after starting new employment
- Download and print the following form from the website and complete it, referring to the example.

Example - Send the following three documents to Rakuten KENPO:
- Application Form for Disqualification as Health Insurance Voluntarily and Continuously Insured Person
- Health insurance card(s) (including those of any dependents)
If you were issued any other documents by Rakuten KENPO (such as an elderly benefits card or a Certificate of Application of Maximum Copayment Amount), return them along with the health insurance card. - Copy of new health insurance card
- Rakuten KENPO will send a Certificate of Loss of Eligibility to your registered address if you so request
(5) If you became an insured person under the Medical Care System for the Advanced Elderly
- Rakuten KENPO will send information to your registered address concerning loss of eligibility and joining the Medical Care System for the Advanced Elderly.
- Promptly after the insured person’s 75th birthday, send the health insurance card(s) (including those of any dependents) and elderly benefits card to Rakuten KENPO.
If you were issued any other documents by Rakuten KENPO (such as a Certificate of Application of Maximum Copayment Amount), return them along with the above documents.- *Note: Dependents can no longer use their health insurance cards from the date on which the insured person lost his or her eligibility.
- Check with the office of the municipality in which you live regarding your new health insurance card.
(6) If you applied to have your status as a Voluntarily and Continuously Insured Person cleared, the last day of the month including the date on which the application was received
- Download and print the following form from the website and complete it, referring to the example.

Example - Send the completed application form to Rakuten KENPO.
- Rakuten KENPO will send a Certificate of Loss of Eligibility to your registered address when the date of loss of eligibility approaches.
- Please return your health insurance card to Rakuten KENPO on or after the day after the date of loss of eligibility.
