I would like to become a Voluntarily and Continuously Insured Person. What procedures do I need to complete?
See below concerning the procedures for becoming a Voluntarily and Continuously Insured Person.
(1) Check whether you satisfy the eligibility requirements.
You must satisfy all of the following conditions:
- You must have lost your eligibility as an insured person of Rakuten KEMPO due to retirement or similar reasons.
- You must have been an insured person for a continuous period of at least two months until the date you lost your eligibility.
- You must apply to become a Voluntarily and Continuously Insured Person within 20 days after the date you lost your eligibility.
*You may also submit the application form prior to the date you leave your employer.
(2) Print and fill out the application form.
- Download and print the following form from the website and complete it, referring to the example.

Example - If you would like to name family members as dependents, also prepare the necessary attachments. (Since the plan for Voluntarily and Continuously Insured Persons differs from your current health insurance, you will need to resubmit the dependent certification documents.)

(3) Send the application form and other documents to Rakuten KENPO
- *The application form must be received by Rakuten KENPO within 20 days from the day after the date you left your employer. Note that applications even one day late will be legally rejected.
- *Only original (paper) application forms will be accepted.Application forms will not be accepted if sent by email in PDF or other format.

- *Rakuten KENPO will contact you directly if there are any problems with the documents you submit. Be sure to enter your mobile phone number and email address on the application form.
(4) Return your health insurance card, Certificate of Application of Maximum Copayment Amount, elderly benefits card, etc. to HR or the person in charge of social insurance at your employer (company) by the date when you leave your employer.
- Your employer will initiate procedures for loss of eligibility after the health insurance card and other documents have been returned.
(5) Your employer will submit the Notification of Loss of Eligibility to Rakuten KENPO.
- Procedures to add you as a Voluntarily and Continuously Insured Person will begin after the Notification of Loss of Eligibility is received from your employer.
- *Depending on the employer, it may take up to two weeks or so to complete the procedures for loss of eligibility.
(6) Pay your first premiums using the statement sent from Rakuten KENPO.
- Pay your first premiums by the deadline indicated on the statement.
- *Second and subsequent premium payments
Deadlines for payment of premiums are specified in advance. You will be sent a statement for payment. Review it carefully and be sure to complete the remittance procedures by the day before the payment deadline. Note that your eligibility will be cancelled if payment is received even one day late.
(7) After confirming the payment of your first premiums, Rakuten KENPO will send you a new health insurance card.
- Your health insurance card will be sent to the address provided on the application form.
- Until a new health insurance card arrives, be sure to inform the staff at the medical care institution that procedures are underway to change your health insurance card and follow instructions provided by the medical care institution.
If you have paid any costs up front, you can carry out the corresponding settlement procedures once your new Voluntarily and Continuously Insured Person health insurance card arrives. (See here for details.)
