See below if you need a Certificate of Loss of Eligibility to join another health insurance society or for other reasons after resigning or retiring from your employer. The Society can issue a Certificate of Loss of Eligibility once it has confirmed your loss of eligibility based on the notification from your employer submitted on or after the date of your resignation or retirement.
1.Complete the resignation/retirement procedures at your company (employer).
2.Print the Request for Health Insurance Certificate. Fill out and submit the form to the Rakuten KENPO/Rakuten Health Insurance Society.

- *Only requests using the original form (paper) will be accepted.
Note that requests using email attachments (e.g., PDF files) will not be accepted.

3.Return your health insurance card, Certificate of Application of Maximum Copayment Amount, elderly benefits card, etc. to HR or the person in charge of social insurance at your employer (company) by the date when you leave your employer.
- *Your employer will initiate procedures for loss of eligibility after the health insurance card and other documents have been returned.
4.On or after the resignation/retirement date, your employer will submit a Notification of Loss of Eligibility to the Health Insurance Society.
- *It may take some two weeks for your employer to complete the procedures to finalize your loss of eligibility. Please check with your employer (company) regarding the status of these procedures.
5.The Health Insurance Society will prepare your Certificate of Loss of Eligibility and send it to your designated address.
Once the Society has reviewed and confirmed the notification from your employer, it will promptly prepare and mail a Certificate of Loss of Eligibility. The certificate will be mailed to the address you indicated on the application.
(The certificate will not be mailed to overseas addresses.)
