What do I need to do to change my name, address, or other information while I am a Voluntarily and Continuously Insured Person?
In the following cases, Voluntarily and Continuously Insured Persons are required to submit the following documents to the Health Insurance Society:
1. Name change
2. Relocation of residence
3. Change in contact telephone no.
4. Change in contact mail address
