Apply to the Health Insurance Society if the name of the insured person or a dependent has changed or you want to add a new family member as a dependent due to marriage or other reason.

- * After you submit the application form, please use your Myna health insurance card. If you cannot do so, such as because you have not had your Individual Number Card issued, please also submit an Application Form for Issue/Reissue of Eligibility Verification Certificate.
