After you leave your employer
You will lose your eligibility for membership in the Health Insurance Society after you leave your employer. Join the appropriate medical care insurance system based on your needs and circumstances.
- Return your health insurance card when you leave your employer
- If you need a Certificate of Loss of Eligibility
- If you wish to remain a member of the Health Insurance Society

- * You can no longer use your health insurance card from the day after the date of leaving your employer.
Note that the former insured person him or herself will be asked to repay all medical care costs incurred for examinations at medical care institutions from the day after the date of leaving employment.

You will need to apply again for dependent family members. Access the application form from the "Family membership" page and provide the information requested. The Dependent Survey Form is not required for family members who became dependents while you were an employee. (You need to attach official documents.) |
